Step 1: Confirm Medicaid eligibility

Most DDD services are funded through Medicaid, so establishing and maintaining Medicaid eligibility is usually the first step. Families who need help can work with a Medicaid navigator or their county board of social services.

Step 2: Request DDD eligibility

Submit an application for eligibility to the Division of Developmental Disabilities, including documentation that the developmental disability began before age 22. DDD reviews the application and notifies you of the determination.

Step 3: Choose a Support Coordination agency

Once eligible, you select a Support Coordination agency. Your support coordinator helps you build a person-centered plan (the NJ Individualized Service Plan) that describes your goals and the services to reach them.

Step 4: Select approved providers

With a plan in place, you choose approved providers to deliver services. This is where Chriswell Health can step in, working alongside your support coordinator to provide compassionate, dependable support.

How long does the DDD application take?

Timelines vary depending on documentation and Medicaid status. Establishing Medicaid and gathering records that show the disability began before age 22 are often the longest steps. Starting early and staying organized helps.
