Common Questions

Quick answers to common questions

Short answers to the questions we hear most, with deeper guides throughout the Learning Center.

How do I know when a loved one needs home care?

Watch for changes such as difficulty with daily activities, missed medications, weight loss, falls, or increased isolation. A nurse-led assessment can clarify what level of support is truly needed.

Who qualifies for NJ DDD services?

Eligibility generally requires being a New Jersey resident aged 21 or older with a qualifying developmental disability that began before age 22, and Medicaid eligibility. A support coordinator guides families through enrollment.

What is nurse-led home care?

Nurse-led home care means a licensed nurse designs and supervises every care plan, monitors for changes in health, and coordinates with your family and physicians, bringing clinical oversight to even non-medical support.

How can I prevent a hospital readmission after discharge?

Reconcile medications, attend every follow-up, know the warning signs, and keep one point of contact who coordinates the plan. Transitional care like BridgeCare is designed to make the first 30 days safe.
