Job Summary:
We are seeking dedicated and compassionate Registered Nurses (RNs) to join our home health care team. In this role, you will provide skilled nursing services to clients in their homes, ensuring they receive the highest quality of care with dignity and respect. You will oversee care plans, perform clinical procedures, and coordinate with physicians, caregivers, and family members to promote the client’s health and well-being.
Key Responsibilities:

Conduct comprehensive nursing assessments for new and existing clients.

Develop, implement, and regularly update individualized care plans.

Administer medications, injections, and treatments as prescribed.

Perform wound care, dressing changes, and post-surgical monitoring.

Monitor vital signs, symptoms, and overall client health status.

Provide education and guidance to clients and families on managing health conditions.

Coordinate with physicians, therapists, and other healthcare professionals.

Supervise and support CHHAs, LPNs, and other care staff.

Maintain accurate, timely, and compliant clinical documentation.
Requirements:

Active RN license in the state of New Jersey (or eligibility to obtain).

Minimum 1 year of clinical nursing experience (home health experience preferred).

Strong assessment, critical thinking, and problem-solving skills.

Excellent communication and interpersonal skills.

Reliable transportation and valid driver’s license.

Ability to work independently and as part of a team.
