For Support Coordinators
We know what makes your job harder. Providers who don’t pick up the phone. Providers who say yes and then can’t staff the case. Sloppy notes that cost you at audit. Surprises you find out about from the family before you hear from the agency.
Our goal is to be the opposite of that. When you send us a referral, you’ll get a real response from a real person, usually the same day. We’ll tell you honestly whether we can take the case before we accept it. Once we start, we communicate. If something goes wrong, you hear it from us first.
We document the way DDD expects us to. We bill cleanly. We don’t disappear. That’s the standard. Hold us to it.
Services We Provide
We deliver services authorized under each individual’s approved budget and Person-Centered Planning Tool. The most common services we provide are:

Individual Supports
One-on-one help with daily routines, life skills, personal goals, and community participation. The day looks different for every person we work with, which is the point.

Community Based Supports
Getting out into the community for volunteer work, recreation, faith activities, classes, or anything else that matters to the individual.

Respite
Short-term support so that family caregivers can rest. We offer respite hourly, daily, or for longer stretches when families need it. It can happen at home or out in the community.

Personal Care Assistance
Help with bathing, grooming, dressing, mobility, toileting, and other daily living needs. Done with respect and discretion.

Companion Services
A consistent, trusted person who shows up, engages, and provides supervision and presence.

Nursing Services
Where authorized and appropriate, our nurses can provide medication oversight, health monitoring, and coordination with the individual’s medical team. Most DDD providers can’t offer this. We can.

How Intake Works
Here’s what happens when we get a referral:
- We respond within 24 hours, usually faster. We review the authorization, the scope, and the schedule, and confirm we can meet it. Then we set up a meet and greet at the individual’s home, on a day and time that works for them. From there, we match the individual with a Direct Support Professional based on schedule, personality, language, faith, and the specific needs of the person being served.
- Services usually start within one to two weeks of referral. Urgent cases move faster when we can.
- You and the family will hear from us along the way. Not just when something is wrong.
Who We Serve
We serve individuals across most of New Jersey, with a primary footprint in Mercer, Burlington, Somerset, and Middlesex counties and steady coverage in surrounding areas. If your individual is in a county and you’re not sure if we cover it, ask. The answer is almost always yes.
What We Promise the Individual and the Family
The plan starts with what the individual wants. Not what’s convenient for us, and not a template we reuse.
We respect culture, faith, identity, and routine. If those things matter to the person we’re serving, they matter to us.
If we ever need to say no to something, we’ll say it directly and explain why. We’d rather be honest than overpromise.
Common Questions
Are services really at no cost to the family?
Yes, when services are authorized through the individual’s DDD budget. We bill the state directly. Families don’t pay out of pocket for what’s authorized.
How fast can services start?
Do you take individuals with complex medical needs?
We’re a nurse-led agency, so we’re often a good fit for medically complex individuals. We assess each referral on its own. We won’t say yes if we can’t safely deliver.
What about behavioral support needs?
Depends on the situation. We’ll talk through the case with you before accepting it. If we’re not the right fit, we’ll tell you and try to point you somewhere that is.
How are your DSPs trained?
All of our Direct Support Professionals complete the required DDD training, including College of Direct Support modules, CPR and First Aid, Danielle’s Law, and our internal onboarding. We continue to develop our team after they’re hired.
Can a family request a specific caregiver match?
Yes. We try to match for language, gender, faith, age, and temperament whenever possible. The right match changes everything.
