Job Summary:
We are looking for caring and reliable Licensed Practical Nurses (LPNs) to provide skilled nursing care to clients in their homes. Working under the supervision of a Registered Nurse (RN), you will assist with medical treatments, monitor client health, and ensure that each client receives safe, compassionate, and high-quality care.
Key Responsibilities:

Assist in developing and following individualized care plans under RN supervision.

Administer prescribed medications and treatments.

Perform wound care, dressing changes, and other nursing procedures.

Monitor vital signs and report changes in client condition to the RN or physician.

Provide health education and guidance to clients and their families.

Maintain accurate and up-to-date clinical documentation.

Support and guide CHHAs and other caregiving staff as needed.

Ensure compliance with all healthcare regulations and standards.
Requirements:

Active LPN license in the state of New Jersey (or eligibility to obtain).

Minimum 6 months of nursing experience (home care preferred).

Strong clinical skills and attention to detail.

Excellent communication and teamwork abilities.

Reliable transportation and valid driver’s license.
